Liability Waiver and Release Form

To ensure we both understand our roles and responsibilities, I ask you to review and agree to the following terms. My priority is your safety and success!

1. Acknowledgment of Risk

I understand that participating in fitness and training programs, whether in-person or online, involves some level of risk, including potential injuries. I choose to participate knowing these risks and agree to follow all instructions to minimize them.

2. Assumption of Risk

I take responsibility for my health and safety during all activities. I confirm that I am physically and mentally capable of participating and have consulted a doctor if needed.

3. Waiver of Liability

To the fullest extent permitted by law, you release Hermes Daniël Fitness, its employees, contractors, and affiliates from any claims, demands, or causes of action arising out of your participation in fitness activities, except in cases of gross negligence or intentional misconduct.

4. Commitment to Safety

Your well-being is my priority. Please share any concerns or health issues so I can adjust your program as necessary.

5. Medical Disclaimer

I understand that Hermes Daniël Fitness Online does not provide medical advice or treatment. For specific health concerns, I agree to consult a healthcare professional.

6. Confidentiality and Data Protection

Your personal information is safe with me and will only be used to support your training.

7. Governing Law

This agreement is governed by German law and disputes will be handled in the courts of North Rhine-Westphalia.
